Neurofilament light chain (NfL) is a structural protein found in neurons, specifically within axons. When neuronal damage or degeneration occurs, NfL is released into the cerebrospinal fluid (CSF) and, at lower concentrations, into the blood. As a result, it has emerged as a sensitive biomarker of neuroaxonal injury.
In neurodegenerative diseases, such as Alzheimer’s disease, Parkinson’s disease, multiple sclerosis, and amyotrophic lateral sclerosis (ALS), elevated NfL levels reflect ongoing neuronal damage.
Importantly, NfL is not disease-specific; rather, it indicates the intensity of neurodegeneration. This makes it particularly useful for monitoring disease progression and treatment response rather than for definitive diagnosis.
